In-Person Medical Coverage Enrollment & Eligibility Assistance

Are you trying to secure medical coverage but don’t know where to begin? Sunset Health is here to help. Our certified and trained team of Eligibility Outreach Workers can provide in-person access to medical care options that best fits your current care needs and your family’s. We can assist you by providing resources, educational material, step-by-step assistance, submitting an application, a renewal or changes to an application. Contact an Eligibility Outreach Worker at your nearest Sunset Health location and schedule your next appointment. We can provide assistance for any of the following programs:

SNAP Supplemental Nutrition Assistance Program
SNAP is a federally-funded program that provides nutrition benefits to millions of eligible, low-income individuals and families so they can purchase healthy food. Individuals receiving SNAP benefits can pay for food with an electronic benefits card (EBT) which looks and works much like a debit card. EBT cards can be used to purchase food at supermarkets, convenience stores, and other locations.

Eligibility for SNAP depends on factors such as your income, the number of people in your household, resources such as bank accounts, and benefits such as Social Security and unemployment. Certain non-citizens, such as those admitted into the US for humanitarian reasons, may be eligible for the program.

AHCCCS (Arizona Health Care Cost Containment System)

The ArizonaHealth Care Cost Containment System (AHCCCS) is Arizona’s Medicaid program, a health care program funded by the federal and state governments for individuals and families who qualify based on their income level. The program provides medical insurance coverage for common healthcare needs, such as doctor visits, physical exams, immunizations, hospital care, and prescriptions. Approximately 1.9 million low-income Arizonans are currently covered under AHCCCS.

In order to qualify for AHCCCS, you must be an Arizona resident who meets the monthly income eligibility requirements. You must be a US citizen or a non-citizen who meets certain criteria.

Health Insurance Marketplace
This service allows individuals to compare various private health plan options based on affordability, coverage provided, and other important criteria. It’s an ideal resource for those who don’t receive insurance benefits through their work. Plans cover essential health benefits, pre-existing conditions, and preventive care. Some plans also include dental coverage. In other cases, free-standing dental plans are available.

In order to enroll in Marketplace health coverage, you must be a US citizen of national, and not be incarcerated. It’s important to remember that Open Enrollment happens only between certain dates. For 2024, those dates are November 1st through January 15th for coverage beginning in 2025.

KidsCare – Arizona’s Children’s Health Insurance Program (CHIP)

This program was created to help families who exceed income limits established by AHCCCS but can’t afford to pay for their children’s health insurance. Under KidsCare, children can receive routine check-ups, immunizations, dental care, and many other services.

To be eligible, children must be under 19 years old, a US citizen or a qualified immigrant and an Arizona resident. Children must be without health insurance coverage for at least 90 days before they can get KidsCare, but here are some exceptions to this rule.

Monthly premiums are based on a family’s income and size and typically range from $10 to $70 per child. Per federal law, Native Americans enrolled with a federally recognized tribe and certain Alaskan Natives are exempt from a monthly premium subject to proof of tribal enrollment.
